New Patient Steps: What to Do Next
Step 1: Schedule Your Appointment
Schedule your new patient appointment through our online portal. Be sure to select whether you’re booking for an adult or a child—this ensures that you receive the correct intake form link. And don’t forget to let us know who referred you—we love knowing who to thank for sending you our way!
Step 2: Complete Your Online Intake Form
Please fill out your intake form at least 24 hours before your first visit. This allows our doctors time to review your health history in advance, so they’re well-prepared to have a meaningful, personalized conversation with you about your wellness goals.
Step 3: Arrive 10 Minutes Early
We recommend arriving about 10 minutes before your scheduled appointment time. When you arrive, you’ll be warmly greeted by our team. One of our team members will show you how to sign in and make you comfortable while you finish any final paperwork. After a brief tour of our office, your first visit will begin, and you’ll be well on your way to better health!
